2. initial phone intake

Initial Phone Intake

After completing the Inquiry Form, you will receive a link to schedule your initial phone intake.

During this 30 minute phone call we will:

Review all REVEL program offerings
Discuss potential client background, needs, interests, and goals
Determine if REVEL is a good fit for the client

After the intake phone call, you will receive:

Link to schedule in-person intake (We require a $50 fee to schedule in-person intakes)
All intake paperwork to be completed prior to in-person intake

3. in-person intake

IN-PERSON TOUR AND INTAKE

Prior to the in-person intake, please complete the following:

Schedule in-person intake (We require a $50 fee to schedule in-person intakes)
Complete all intake paperwork

The in-person intake meeting includes:

Interview with potential client and parent/guardian
One hour observation in the REVEL environment without parent/guardian present (This does not apply to potential clients interested in the REV UP Employment program or Mental Health Program).
