initial phone intake
Initial Phone Intake
After the intake phone call, if services are deemed a match, we will confirm available funding with your case manager or confirm private pay for services.
If we are actively enrolling in the program you are seeking, you will receive the following:
- All intake paperwork (must be completed before scheduling the assessment)
- A link to schedule the in-person assessment
If we are not actively enrolling in the program of interest, we will add you to our waitlist for assessment. Once enrollment reopens, you will move forward with the steps listed above.

in-person Assessment
IN-PERSON TOUR AND INTAKE
Prior to the in-person assessment, please complete the following:
- Complete all intake paperwork (once completed, we will move to scheduling the assessment)
- Schedule the in-person assessment
The in-person assessment includes:
- Interview with the potential client and parent/guardian
- One-hour observation in the REVEL environment without parent/guardian present
(This does not apply to potential clients interested in the REV UP Employment Program or Mental Health Program.) - REVEL Program Fit Assessment
